If any of those mothers in the area noted above are going to be officers in the new chapter, please note that, too.

I, the undersigned at-home mother, wish to start a local MOMS Club® for the at-home mothers in my community. This MOMS Club will be operated in a nonprofit manner and shall not be used for business, political or religious purposes. I want this chapter to be included under the International MOMS Club group nonprofit status. Enclosed is a check for $30 (in US Dollars) for the new chapter's first year's fee.

(Allow 6-8 weeks for delivery. $30 registration fee not refundable due to the cost and high quality of materials sent. The sponsoring chapter may pay the registration fee for their new sister chapter from their treasury. Chapter manuals not in use by ongoing registered chapters must be returned to the International MOMS Club. All chapter names and geographic areas must be approved by the International MOMS Club. Prospective chapters may not begin meeting separately from their sponsoring chapter or advertise until their registration has been accepted!
